Submit a Referral
Fill out our short referral form for either infant or pediatric services. It only takes a few minutes and helps us prepare for your child's specific needs.

Quick answers to the questions we hear most often from new families.
What age range do you accept?
We currently accept new patients under age 4. Some Medicaid plans limit acceptance to under age 3. Infant services (Sunny Baby) are specialized for newborns through 11 months.
Do you accept my insurance?
We accept Humana Medicaid, Children’s Medical Services (CMS), Sunshine Health (ages 0–3), Simply Medicaid (Early Steps only), and the Early Steps Program Contract. Private insurance for infant feeding is available through The Lactation Network and Wildflower Lactation. Private pay is also welcome.
Where do therapy sessions take place?
Most Early Intervention services are provided in your home or daycare. We also offer in-office sessions at 1738 Thomasville Rd., Tallahassee, and teletherapy via video. Travel fees apply for home/daycare visits.
How do I get a referral?
Just fill out our referral form above — no physician referral is required for most services. For Early Steps, a referral from your pediatrician or self-referral may be used.
How quickly can we get started?
After submitting your referral form, our office will contact you within 1–2 business days. Availability varies but we work hard to get evaluations scheduled as quickly as possible.
